The community compensates us through a referral system. Because we are independent, Graceful Transitions Senior Advisors chooses communities based on safety and client suitability, not contractual obligations. We only recommend communities where we would confidently place our own family members.

  • An active adult community is a retirement or independent living space that offers a resort-like lifestyle with ample amenities. It provides numerous activities and opportunities for socializing, both onsite and offsite.

  • On-site personal and medical care are not available. However, light housekeeping and various activities for an active lifestyle are provided. Additionally, living plans may include 1-3 meals per day.

  • Residential senior living offers a secure and vibrant community, along with assistance for daily living activities. Included are meals and personalized care plans, as well as a calendar of social events, all supporting a relatively independent lifestyle.

  • Specialized residential care is available for individuals with Dementia, Alzheimer's, and Parkinson's disease, including 24-hour supervision and assistance with daily living activities. Additionally, socialization and activities are tailored specifically for residents with dementia.

  • An inpatient facility that offers both short-term and long-term skilled nursing care, along with rehabilitation services. It provides 24-hour medical support and additional medical services are available on-site at all times.

  • Respite care is a short-term care service that provides temporary relief for primary caregivers giving them time away or time to rest.

    • Many long-term care communities offer respite care for a daily rate.

    • Respite care can be offered in-home with caregivers.

    • Some hospice services offer respite care services in a skilled nursing facility on an intermittent basis.

  • Community-based services that provide assistance with activities of daily living (ADLs), personal care, and supervision of adults. Many times, this is offered for people with dementia and can help reduce stress on caregivers.

  • The supportive living program is an alternative to nursing home care for low-income seniors 65+ and adults age 22-64 with disabilities under Medicaid. These offer personal care services where residents can live independently with the help of ADLs. Meals, laundry and weekly housekeeping are included.

    Medicaid Memory Care is available in select Supportive Living facilities and Nursing homes, but not all of them.

    Medicaid Nursing homes often include the same amenities as Supportive Living facilities, but with a much higher level of care.

    Once approved for Medicaid, patients are liable for monthly rent in a Medicaid facility. This is called Patient Liability. Patient Liability is monthly income, minus allowable deductions, and the personal needs allowance. The remaining amount is paid directly to the facility
