Senior Living Placement Services

Deciding to leave a beloved home is a big step and should be approached with care. Feeling at ease can make this decision much easier to navigate.

Step 1: We start with a consultation to learn about your preferences, budget, location, and healthcare needs. By getting to know each person, we tailor our search to fit perfectly. Our services accommodate all budgets and situations.

Step 2: We take care of all the initial research and communication, making the process smoother and ensuring you visit only communities that meet your needs.

Step 3: We book your appointments, join you on tours, ask the right questions, and offer support when you need an advocate the most.

Step 4: We're here for you every step of the journey, from beginning to end and beyond. We help you advocate, understand your rights, and support your loved one with confidence, knowing you've made the best choices along the way.

This Service is completely FREE for you! The community compensates us through a referral system. Because we are independent, Graceful Transitions Senior Advisors chooses communities based on safety and client suitability, not contractual obligations. We only recommend communities where we would confidently place our own family members.
